Program Description

Only those 5 years of age and older are eligible for the Foundations Program

All SFM Providers are “Out of Network Direct Pay Providers”

All care overseen directly by Dr. Potter

60-minute functional medicine intake history and physical with NP – $500
First Follow -up visit in the first 3-6 months – 30 minute-visit with NP or 45 minutes with NP (30) and MD (45)

This does not include Urgent Care as Sanctuary is not equipped for urgent care needs. You will need to designate at urgent care option for after hours and urgent needs.

2 Options for ongoing care:

Pay per visit option is default option unless you choose to enroll on the ELP at some point
Pay-Per-Visit follow-up option- $230 per 30 minutes with NP or $280 per 30 minutes with MD (based on availability). Limited portal messaging. Limited discounts on labs/supplements.
Empowered Life Program – monthly subscription $110/month. Required for those on controlled substances. Full lab and supplement discounts, two-way messaging on portal, every 6-month wellness visit with NP, discounts on other visits.

Lab costs separate from visit cost. Your insurance may cover labs. See below for details.

Enrollment Process

Enroll in our online patient chart – MDHQ (link here)

Sign the Foundations Contract in the questionnaire section or the portal
Email PSC that you have completed your contract
PSC will enter invoice for deposit on your chart and email available appointment times
Pay the deposit in the chart
Email PSC with first and second choice appointment times
PSC will email you with confirmed appointment and instructions for filling out forms before your first visit

Further Explanation of Foundations Program

Establishing an ongoing trusted relationship with a functional doctor is becoming more important each day. To fill this missing piece in your health journey, Sanctuary serves as a Functional and Wellness Specialist and not a Primary Care Provider. We give you what you cannot get with your primary care provider, and we work to limit your need for them by preventing health problems with functional lifestyle changes. If you have acute health problems, we provide the natural alternatives so you can minimize pharmaceuticals. This program is available to those who are 5 years of age and older.

We encourage people who want a Functional health care team working with them to enroll before an acute episode. We schedule out our new patient appointments a few weeks, and we generally cannot take care of urgent acute issues of non-patients in less than two weeks.

However, we do have room for acute visits for our established patients. Because we already have an established relationship, we can take care of many sick or acute visits via telemedicine thereby making life easier for you. If you need to come into the clinic, you can do so.

Foundations Program: Entry Point to Everyday Functional Care

This Foundations Program allows you to begin your functional care relationship with us through a 60- minute in depth Functional Medicine history and physical. This visit includes time with one of Sanctuary’s Nurse Practitioners and is reviewed and directed by Dr. Potter. The parameters for the second visit are outlined below.

Due to their complexity, some specialty testing requires specific visit times for full review and explanation. These details are outlined on our website. In addition, the Foundations Program gives you access to Sanctuary’s IV services. Just let us know your interest so that the provider can order the appropriate screening lab work to make you fully eligible.

At any time after this first visit, you can determine if you want to continue as a pay per visit patient or enroll in the monthly Empowered Life (ELP) subscription program. At any time, you can elect to enroll in the ELP Program. Pay per visit is the default option.

Hormone Balance

Sanctuary’s Foundations Program is a good fit for those looking for hormone help in the context of a whole person health perspective. With this perspective, we reach for the goal of maximizing your body’s ability to produce its own hormones at an appropriate level, not just relying on ongoing bio-identical hormone replacement. If we can get the body healthier, the hormones may eventually take care of themselves. In the meantime, we can give appropriate hormone replacement and support. If ongoing replacement is necessary, we can provide that naturally.

Option #1 – Pay Per Visit

Should you continue with the pay per visit option, the second visit will be determined as follows. If any therapy is started or testing performed, then you will need to return at the time and for the visit duration determined with your provider. If no therapy is started and no testing is performed, this second visit can be anytime in the next 6 months. After this second visit, other visits are determined as follows. If you are on a prescription medication managed by SFM then you must be seen every 6 months at a minimum. If you are on supplement therapy, you must be seen at least once a year to remain a patient at SFM.

As a pay per visit patient, you can generally choose to schedule with either the Nurse Practitioner or Dr. Potter based on desire and provider availability. Dr. Potter’s visits are scheduled further out. As a pay per visit patient, you have limited access to the patient portal (no two-way messaging) and you get 15% discount on any supplements ordered through our recommended supplement store. Your non-insurance-covered labs will generally be slightly less than retail but not at the full discount offered to full program patients. However, if your insurance is contracted with Lab Corp and/or Quest, those companies work out of our office. Any labs sent to them will be filed with your insurance. You can get invoices to submit to your insurance for lab reimbursement should it be needed. We cannot guarantee reimbursement.

Option #2- Empowered Life Program

With the Foundations Program, you can elect to enroll in the Empowered Life Program (ELP) any time after the first visit. This monthly subscription program gives you full portal access, full discounts on labs, full discounts on supplements, and one wellness visit every six months with a Nurse Practitioner at SFM. You can schedule the every-six-month visit with Dr. Potter for an extra $50 per visit. The first included 6-month visit will be scheduled from the date you sign up for the ELP and this visit cannot be moved up. You can schedule additional visits between the 6-month check-ups at a discounted rate.

Those enrolling in the ELP subscription directly after the first visit who have started on a therapy will need to come back for a follow up at the time determined with your health provider. If you are a part of this program, the cost of the visit is discounted but length of the visit and the provider seen will be determined by clinical need.

Specialist Care Not Primary Care

With either the Pay Per Visit option or through our Empowered Life Program, you have access to Sanctuary functional care for your foundational health needs and maintenance. Again, we are not your primary care provider. You need a plan for accessing conventional urgent care for things such as suturing and emergency care. If you desire the conventional recommended health screenings or well child exams, you will need another physician to manage these needs. In addition, Dr. Potter and the team are functional specialists and cannot be listed as your primary care providers. Access to the Sanctuary team in this program runs from 7am to 6pm daily. After-hours calls have an additional charge.

First follow up visit within 6 months- $230 for 30 minutes with NP or $345 for 45 min with NP (30) and MD (15)

Remaining Pay-per-visits: $230 per 30-minute visit with NP and $280 per 30-minute visit with MD

Empowered Life Program (ELP): $110/monthly subscription includes 2-way portal messaging, full lab and supplement discounts, and an every-6 -month wellness visit with the NP. Extra visits are discounted at $180 per 30-minute visit with the NP and $230 per 30-minute visit with the MD.
