SFM Clinical Emergency Protocol

Symptoms to Initiate Clinical Emergency Protocol and Notify Provider Immediately: 

1. Hypotension

    • Blood pressure drop by over 20 points from initial BP

2. Allergic Reaction?

3. Respiratory Distress

    • Wheezing
    • Difficulty swallowing
    • Decreased O2 saturation – Sats under 92%
    • Increased respiratory rate

4. Chest Pain

5. Change in Mental Status


Clinical Emergency Protocol:

Hypotension: Blood pressure drop by over 20 points from initial BP

  1. Immediately call for nursing help and notify MD
  2. Lay patient back in chair/exam table
  3. Be ready to start IV per MD instruction and prepare Normal Saline 500ml bag for infusion
  4. Obtain Vital Signs every 5 minutes


Respiratory Distress
with any of the following symptoms: difficulty breathing, wheezing, difficulty swallowing, decreased O2 saturation (sats under 92%) and/or increased respiratory rate

  1. Immediately call for help and notify MD
  2. Retrieve AED, oxygen, and go-bag and instruct 911 to be called
    • If oxygen is 92% or less apply oxygen at 2L per nasal cannula
  3. If patient becomes unconscious and stops breathing, initiate ACLS protocol
  4. If patient is conscious and still breathing on their own:
    • Obtain VS, and continue every 5 minutes
    • Follow Provider orders


Chest Pain

  1. Immediately call for help and notify MD
  2. Assess patient for symptoms of
    • Pain that radiates to jaw, arm and back
    • Nausea/vomiting
    • Sweating
    • Shortness of breath
    • Hypertension
  1. If showing signs of Acute Coronary Syndrome initiate ACLS protocol and call 911

 

Change in Mental Status

  1. Immediately call for help and notify MD
  2. Obtain Vital Signs
  3. Retrieve AED, oxygen, and go-bag
    • If oxygen is 92% or less apply oxygen at 2L per nasal cannula

  3. DepoMedrol IV – (Andemeds)
  4. Albuterol 0.65 and 1.25 nebulizers – (Andemeds)
  5. Zofran IV/PO SL – (Andemeds)
  6. Pepcid IV/PO tab – (Andemeds) ***IV FORM IS IN FRIDGE IN NURSE’S STATION
